Intrauterine Insemination by husband sperm (IUI)

WHAT?

Intrauterine Insemination is an assisted, simple, non-invasive, cost effective and first line conception treatment for some patients. It can be termed as “FIRST LINE TREATMENT” for infertility. The procedure involves depositing of “washed” semen sample in the upper part of the uterine cavity by a thin cannula/catheter, thus, shortening the distance the sperm has to travel to reach the egg.

IUI is performed within 90 minutes of the semen sample being ejaculated and submitted. Therefore, it is less time consuming, painless and performed without anaesthesia.

The procedure includes Controlled stimulation of follicles (eggs) by oral drugs or injectable, follicle study (growth/ progress of eggs), timed rupture and timed insemination of sperm.

HOW?

The patient should visit the doctor on the first or second day of her period. Stimulation medication will be prescribed, which should be taken as advised without missing or skipping any dose. She has to follow up after every 2 or 3 days for follicle study (eggs). Once the follicle (egg/s) grows as per the desired size, injection for the rupture of the follicle will be given and the time for IUI will be informed to the patient.

ON THE DAY OF PROCEDURE:

The couple will report on the given time to the clinic. The husband will be told to produce a semen sample in the clinic or at home if you stay close to the clinic. The semen sample should be submitted within 30 minutes of ejaculation.

The semen sample is kept for liquefaction for 30 mins after ejaculation. Semen analysis is performed which will grade your sperm for concentration (number of sperm) and motility (ability of the sperm to displace itself). Depending on the report, the semen sample is processed which will result in separating total motile sperm from the ejaculate. These sperm can now be inseminated into the uterine cavity.

Meanwhile, the patient is asked to make a full bladder for easy access to cervix and thus the smooth passage of the catheter into the uterine cavity.

POST IUI CARE:

The patient is allowed to rest for 15-20 minutes post insemination and discharged. It is advised that the couple keeps coitus post insemination. The drugs given post procedure should be consumed accordingly. There is no compulsion of complete bed rest, dietary implications or any extra care after IUI. Lots of fluid intake is advised. One should see to it that they do not contract cold, cough, fever post procedure.

FOLLOW UP:

If the period date is skipped, a urine pregnancy test at home can be performed. The patient has to inform or visit the clinic regarding the same for which an appointment for your visit will be scheduled for further check-up.

If the patient gets her period, she has to inform/visit the clinic on first or second day of the cycle. Patients opinion will be taken under consideration to whether continue, stop or change the line of treatment.
